Below are various forms and web links to answer your questions or help prepare for an event you'd like to have at the church. All events must be submitted in advance and approved by the appropriate staff member before plans are made for the event.

Church Use Form - This form should be completed and turned in to the office to request the use of any church area (other than the multi-purpose building).

Multi-Purpose Form - This form should be completed and turned in to the office to request the use of the multi-purpose building (old sanctuary).

Small Group Map - A map of the various locations for our Sunday morning small groups (Sunday School).

Wedding Policy - The general church policy on usage of church facilities and fees.

Wedding Contract - To be completed and returned to the First Baptist Church wedding coordinator.
